Application For Residence

Supervised Community Residence

All placement in our Supervised Community Residences must be approved and presented by a social worker from the Disability Support Program. An assigned case manager (nurse; social worker; psychologist…) must fill out the request for admission (description of present situation, case history, present need of support for residence living).

Requests for admission are studied by the Admission and Discharge Committee which meets as needed. The committee’s decisions are reached according to the urgency of the situation, the client’s needs, and the availability of space to meet these needs.

Admissibility

  • Aged 19 or over
  • Should be in good physical health
  • Shows an interest and/or ability to live independently (with minimum supervision)
  • Open to suggested activities and life skills training and accepts to sign contract
  • Has an assigned case manager (professional from Mental Health field)
  • Diagnosed with a mental illness

Independent Living

All qualified individuals must submit an application and forward it to Alternative Residences Alternatives’s Community Resource Manager (hrh@arainc.org). All applicant’s forms will be reviewed and qualified individuals will be placed on a waiting list.

Once a bed/apartment becomes available, candidates will be contacted for an interview. The committee’s decisions are reached according to the urgency of the situation, the client’s needs, and the availability of space to meet these needs.
